What is the Seniors I.Q.
Program?
The
Seniors Program offers a coordinated and community based
approach that includes physicians, community agencies,
home health staff, social workers, family members and
volunteers to work as a team to IMPROVE THE QUALITY OF
LIFE FOR SENIORS 65 and OLDER. The overall goal of the
Seniors I.Q. Program is to provide FREE AFTERCARE
SERVICES to help seniors continue Living Independently
in their own homes.
Benefits of the
Program
The program will assist
Seniors with the following:
- Develop
individualized, Holistic Plan of Care that will
improve seniors physical, spiritual and social well
being
- Perform home
assessments that will reduce safety risks for
seniors in their homes
- Connect seniors with
Medicare/Medicaid and other health care services
- Assist seniors with
resources and services available to them in their
community
Our Mission:
To improve the overall
health of the communities we serve as part of a network
of health and supportive services that meets health
needs, promotes positive lifestyles, ensures a healthy
environment, and provides useful health information and
guidance.
